We welcome you to Millersburg Veterinary Hospital. If you would like to fill out your forms prior to arrival, you may print it out and bring it with you, or you may fill it out online and send it to us. Please do this at least 24 hours (1 working day) prior to your appointment.

If you are new to our practice, please fill out the New Client form. (We will get your check writing information when you come in if you choose to write a check, please leave this blank now).

If you are already part of “the family,” but have a new pet, please fill out the New Patient form.

Please bring a copy of your pet’s medical records, or arrange to have them faxed to us (573-592-8723) prior to your appointment. Grooming dogs will not be admitted without valid rabies immunization.
